Jacob M. Medina, 19, of Gilbert, AZ was called to heaven on Wednesday, May 13, 2020. He is survived by his parents Mike & Krystal as well as his younger brother Casey. He is also survived by his grandparents, aunts and uncles, and numerous cousins.

Jacob led a very active, happy & healthy lifestyle and he had an avid love for the game of baseball, both as a player and a fan.  He also loved music, cars, shoes, and he was a friend to everyone, in all walks of life. To know him was to love him. Jacob’s bright smile and magnetic personality will be missed dearly by everyone he touched and will remain forever in our hearts.

 

Visitation is Thursday, May 21, 2020 from 6:00 pm – 9:00 pm at Superstition Manor, 1220 North Signal Butte Road, Mesa, AZ 85207.

Catholic mass will be Friday, May 22, 2020 at 10:00 am at Saint Juan Diego Roman Catholic Church, 3200 South Cooper Road, Chandler, AZ 85286.

Burial will be Friday, May 22, 2020 at 11:30 am at Gilbert Memorial Park, 2100 East Queen Creek Road, Gilbert, AZ 85297

Celebration of life reception will follow the burial at Superstition Manor at 3:00 pm.
