Ophelia S Montoya, 74, of Chandler, AZ passed away at home on Sunday, August 23, 2020. Her life was devoted to her family and loved ones.
She was predeceased by her husband, Jim Montoya and her parents Rafael and Cristina Avina.
She is survived by her children Hugo Montoya and April Wooten (Greg). Her two grandchildren, Madison and Drew Wooten. Her sisters Irma Munoz Avina, Nora Avina, Elva Deleon, and Velma Sotomayor, and her numerous nieces and nephews.
The funeral service will be held on September 15th, 2020 at 9:00 a.m. at Forest Lawn Cemetery, 4471 Lincoln Avenue, Cypress, CA 90630.
